Add missing table in upgrade script

Move all pages for subscription member in adherent/subscription (Card / Info / List)
Rename llx_cotisation in llx_subscription
Rename all db field cotisation in subscription
Rename class Cotisation in class Subscription

Fix date of supplier proposal missing.

(sometimes we need, sometimes not).

Fix: a minor error was locking the full upgrade process.

Try to continue work on position of menu entries.
